Your Rights as a Client
Your safety, dignity, and autonomy are important. Texas law requires that I share the following information so you are aware of your rights and the options available to you as a consumer of mental health services.

Accessing Your Health Care Records
You have the right to request access to your health care records at any time. If you would like to request your records, you may submit a written request by email to me at contact@caseylpc.com. Requests are handled with care and in accordance with state and federal laws. If you have questions or would like support with this process, please feel free to reach out—I am happy to help.

Contacting the Texas Behavioral Health Executive Council
If you would like additional information about professional licensure or the regulation of behavioral health providers in Texas, you may contact the Texas Behavioral Health Executive Council (BHEC). Sharing Concerns or Filing a Complaint
If at any point you have concerns about your care or believe your rights have not been respected, I request that you first discuss this with me. If you are not satisfied, you have the option to file a consumer complaint with the Office of the Texas Attorney General’s Consumer Protection Division.
